Grooming Release Form

We want your pet's grooming experience safe and enjoyable! This form outlines a number of considerations that could impact the grooming. We want you to be aware of all of these.

You will only need to sign this form once, and it will apply to all pets you already own as well as those you may acquire in the future. Please reach out by text or call to 469-481-8900 if you have any questions!

Health or Medical Issues

Grooming can sometimes reveal a concealed medical condition or aggravate an existing one. This might happen while grooming or afterward. Please notify us of any current or previous problems so that we can keep an eye out for them. When you check the box here, the pet's owner will bear all medical fees for veterinarian care.

Accidents

Although accidents are extremely rare when working with animals, they do occur. Grooming equipment is sharp, and while we employ extra caution and care in all cases, cuts, nicks, scratches, quickening of nails, and other issues are conceivable when an animal is wiggling or moving around. In the unlikely event that an accident occurs, you will be alerted. If we believe the situation is critical and the owner is unavailable, we will seek immediate veterinary care for your pet at the nearest veterinarian clinic at our cost.

Authorization from a Veterinarian in Case of a Medical Emergency

In the event of a medical emergency while in our care, this release grants us full authority to seek medical attention from the nearest vet clinic.

Vaccines

We ask that your pet be in compliance with your veterinarian’s recommendations for your pet. We also ask for a copy of your most recent vaccination records.

Refusal of Services for Dangerous or Aggressive Animals

At any time, we reserve the right to refuse any service. We reserve the right to refuse grooming services, suspend grooming services, or cancel grooming services at any time before, during, or after grooming. The client will be charged a grooming fee if your pet is too agitated or becomes unsafe to groom (for what was done up until that point).

Muzzles

We don't muzzle unless muzzling is indicated by your pet’s behavior and other techniques of calming your pet have failed. Muzzling your pet is very harmless, and it protects both the pet and the groomer. Muzzling a scared animal can sometimes calm it down, allowing the grooming process to continue. The muzzle also prevents the pet from accidentally licking the scissors or other grooming tools. If your pet bites continually at the tools, the dryer, or the groomer, we reserve the right to terminate grooming and charge a service fee. Dogs who require muzzling or additional employees to manage will be charged at a higher fee than usual. This is due to the fact that grooming aggressive or frightened dogs takes significantly longer.

Interruptions During Grooming Services

We request that you do not interrupt the groomer during grooming services for the safety of the animals being groomed as well as the professional pet groomer.

Matting

Animals with matted coats need special attention. Mats in an animal's coat can become so tight that they harm and tear the skin, providing a breeding ground for infections. Due to skin growths trapped in the mats, removing a densely matted coat might result in nicks, cuts, or abrasions. Heavy matting can also trap moisture and urine near the pet's skin, causing mold, fungus, or bacteria to grow. Brushing can further irritate the skin. Itching, skin redness, self-inflicted irritations or abrasions, and hair failure are all possible side effects of mat removal techniques. Pets may also experience agitation or short behavioral changes in some situations after having mats removed.

Mats are difficult to remove and the groomer may need to shave the pet’s coat entirely. If your pet is matted, we require that you schedule services on a regular interval, usually between 4-8 weeks. By checking the box here, you recognize that your pet may need to be shaved to remove matting, and that you understand the risks involved. This method will incur an additional, per-minute fee because it is slow, careful, time intensive work.

Occasionally, de-matting a pet may take two, three, four or even five hours. Your groomer may not be able to complete the work in a single session. If this is the case, the groomer will return your dog to you with as much of the groom completed as possible and the office will reach out to schedule the groomer to come back out – usually within the week.

Undercoat and Shedding

Every dog sheds throughout the year, though some shed more than others. Some dogs also have a twice-yearly natural "blow out," which can be excruciating for them. Our low-shed treatment employs a number of tools and solutions, depending on what works best for each dog's coat—the Furminator tool, for example, is ineffective on all coat types and cannot be used more frequently than monthly.

We offer (and suggest) one extra "brush only" session within 7-10 days before your dog’s appointment to get the very best results when your dog is going through a period of heavy shedding.

Any de-shed treatment will help considerably with your dog’s shedding but will not stop all shedding completely.

Cancellations/No Shows

You will get a confirmation message 72 hours before your appointment which gives the date and arrival window of your grooming appointment. If the date or arrival window does not work for you, please let us know as soon as possible. All cancellations or reschedules require at least a 24-hour notice to provide us time to fill the spot with a customer from our waiting list.


On the day of your appointment, the groomer will text you their ETA (estimated time of arrival). Grooming appointments will be held for 15 minutes after the stated arrival time. Appointments that are more than 15 minutes late or cancelled with less than a 24 hour notice will be charged a 50% cancellation fee to get back on the grooming schedule.

Satisfaction

Your satisfaction is extremely important to us. If you are dissatisfied with your groom for any reason, please contact us within 48 hours and we will organize a time for a groomer to return for a free adjustment.

Photographs

By signing this release form, you give us permission to photograph your pet for use in customer files, as well as for the company's website and social media pages.

Anal Glands

External gland expression is included in our standard grooming services. With an external expression, the anal glands are squeezed from the outside of the rectum and the liquid is expressed. Please be aware that with thicker anal gland liquid or where an infection is present, this method may be insufficient to completely empty the glands. The ideal way to express anal glands is by internal expression under the care of your veterinarian. Only your vet can express the internal glands.

Acknowledgement

I affirm that I have reviewed this Grooming Release Form for accuracy and understand the contents of this contract. I affirm that I am the rightful legal owner of the dog(s) for which services are being rendered. I authorize this contract to be valid approval for future grooming services, permitting us to accept telephone reservations for service without additional signed contracts or written authorization. I understand that pricing is subject to change. This agreement is valid for all pets that I currently own or will own in the future. I have read, and agreed to the above. I understand that checking this box constitutes a legal signature confirming that I acknowledge and agree to the above terms.
