Monthly Mobile Grooming Membership Agreement

This Membership Agreement is entered into between Backyard Pet Services and the undersigned client as of the date signed below.  The membership agreement is separate from the grooming release and both must be executed and returned in order for services to begin.  If there are multiple dogs in the family, a membership agreement is required for each dog.

1. Membership Requirements

• Client must have a valid credit/debit card on file at all times.
• Memberships are billed on a monthly auto-charge basis to the card on file.
• Memberships require a 12-month contract commitment.

2. Membership Terms

• Membership Benefits - frequency. Your pet’s membership fee includes unlimited grooms for the pet listed on the agreement. Your pet’s grooming is unlimited in the sense that there is no per-groom charge, but scheduling is subject to standard grooming intervals for pet health.  For optimum pet coat health, full grooms should be performed no more frequently than every 4 weeks and baths no more frequently than every 2 weeks.
• Membership Benefits – priority.  Families with grooming memberships receive preferred access to booking but requests do not override existing bookings and are booked based on groomer/grooming van availability.
• Membership Benefits –duration and services.  Appointments are based on a standard grooming time of approximately two hours in length.  Excessive matting, behavioral handling, specialty styles, or additional labor required to groom the dog may incur an additional charge.
• One membership per pet. Each pet must have its own membership.
• Early Cancellation Fee: If Client cancels before completing the 12-month term, a fee equal to one month’s membership fee will be charged.  If cancellation is due to the death of the pet, the early cancellation fee is waived.
• Advance booking. Appointments can be booked up to 45 days in advance and additional appointments become available once the current appointment is completed
• Renewal. Memberships are reviewed at renewal and may be adjusted or discontinued if usage significantly exceeds normal grooming patterns.  Otherwise, memberships automatically renew monthly unless canceled in writing after the initial 12-month term.

3. Payment and Tips

• Monthly payments will be automatically charged on the below date each month.
• Tips may be given directly to the groomer via:
• Cash
• Zelle
• Venmo
• Card-charged tips can be set up on an auto-charge basis each month upon Client’s request.

4. Cancellation and Rescheduling

• Early cancellations are subject to the Early Cancellation Fee stated above.
• We do reserve the groomer’s time and the van’s utilization for your booked appointments.  Appointments that are canceled or rescheduled within 24 hours of the appointment may result in a $25 missed appointment fee (at the Company’s discretion).

5. Contract/Release Form

By signing below, the Client:

• Acknowledges and agrees to all terms listed in this Agreement.
• Authorizes Backyard Pet Services to charge the card on file for monthly fees, early cancellation fees (if applicable), past-due amounts, and authorized tips.
• Releases Backyard Pet Services, its owners, and employees from any liability related to grooming services performed under this membership.
