We want your pet's training experience safe and enjoyable! This form outlines a number of considerations that could impact the training session. We want you to be aware of all of these.
You will only need to sign this form once, and it will apply to all pets you already own as well as those you may acquire in the future. Please reach out by text or call to 469-481-8900 if you have any questions!

I, the undersigned owner, understand that participation in dog training services carries certain inherent risks, including but not limited to injury or illness to myself, my dog, other persons, or other animals. I acknowledge that my dog’s behavior is ultimately my responsibility, and I agree to hold harmless and indemnify Backyard Pet Services its owners, trainers, employees, and agents from any and all claims, damages, or expenses resulting from participation in training sessions or related activities.
I understand that even well-trained animals may act unpredictably, and I accept all risks associated with dog training.
If your dog has a history of biting or tries to bite the trainer during a session, a Bite Fee training of $20 will be added to your current and/or future training sessions.
By signing this release form, you give us permission to photograph your pet for use in customer files, as well as for the company's website and social media pages.
We ask that your pet be in compliance with your veterinarian’s recommendations for your pet. We also ask for a copy of your most recent vaccination records.
At any time, we reserve the right to refuse any service. We reserve the right to refuse training services, suspend training services, or cancel training services at any time before, during, or after training.
To secure your appointment, a valid credit or debit card must be kept on file. This card information is stored securely through our payment processing system and will only be used in accordance with our payment policy.
Payment is due at the time of service. We accept Zelle, Venmo, cash, check, or card. If payment is not received within five (5) days of the completed appointment, the card on file will be automatically charged for the full balance due. There is a 3% transaction fee for card payments.
By scheduling an appointment, you authorize Backyard Pet Services to store and charge your card on file in accordance with this policy.
I affirm that I have reviewed this Training Release Form for accuracy and understand the contents of this contract. I affirm that I am the rightful legal owner of the dog(s) for which services are being rendered. I authorize this contract to be valid approval for future training services, permitting us to accept telephone reservations for service without additional signed contracts or written authorization. I understand that pricing is subject to change. This agreement is valid for all pets that I currently own or will own in the future. I have read, and agreed to the above. I understand that checking this box constitutes a legal signature confirming that I acknowledge and agree to the above terms.
